The final project will require students to develop a disease management plan of care for a patient with a pulmonary disorder. The student can pick any pulmonary disease process that they want. Students will be required to develop a video presentation that outlines their plan of care for the patient utilizing the elements covered in this course and found in the literature. Students should act as though they are teaching the patient and their family. Students will be presenting this from the perspective as they are part of the larger inter-professional team and have been asked to educate the patient and their support system (as applicable). Attention should be made towards addressinghealth barriers, promoting self-efficacy, education on the plan of care and self monitoring. The student should outline all areas of disease management for the patient and specifically focus on the role of the respiratory therapist in this. Students must have a minimum of 3 peer reviewed references to support the information used to develop the disease management plan of care. You must format your references using APA style. You are only required to list your references on an uploaded Word document, you do not have to cite them in your presentation. It is understood that you will use the references to support the development of your treatment plan. Students may not submit a voice-over PowerPoint. A video is required for this assignment. This video should be 15 minutes long. Use the below "Basic Interview Expectations" as a guide when building your interview.
