Identify the 10 steps you would take to insert Ms Warral’s IDC by placing them in the correct order 1-10. Correct order Steps • Cleanse the urethral meatus. • With the nondominant hand, fully expose the urethral meatus by spreading the labia. • With the dominant hand, grasp an antiseptic-soaked cotton ball with forceps or pick up an antiseptic swab stick • Wash the perineal area with soap and water, locate the urinary meatus, and dry the area. • Use light source to illuminate the perineum as needed. • Discard used supplies, remove gloves, and perform hand hygiene. • Place a waterproof pad under the patient. • Assist the patient to a dorsal recumbent position (supine with knees flexed). • Ask the patient to relax her thighs to externally rotate the hip joints. • Maintain privacy • Inflate the balloon of the indwelling catheter with sterile water to the volume indicated in the manufacturer’s instructions. Do not inflate the balloon until urine flow appears; early inflation may injure the urethra. • Gather equipment • Perform hand hygiene. Don PPE • Introduce yourself to the patient. Verify identify. • Explain procedure, gain consent. • Raise the bed to an appropriate working height • After inflating the balloon, pull gently on the catheter tubing until resistance is felt. • Place the drainage bag below the level of the bladder • Set up the indwelling catheterization kit using sterile technique. Refer to TAFE Queensland intranet for current version of document. 14 Document1 • Clean the perineal area, wiping front to back from the clitoris toward the anus. • Use a new cotton ball or swab for each area cleansed. • Wipe the far labial fold first, the near labial fold next, and then directly over centre of the urethral meatus • Remove PPE and perform hand hygiene. • Assist the patient to a comfortable position. Lower the bed and return the side rails to the original position. • Discard supplies and perform hand hygiene. • Document the procedure in the patient’s record • With the sterile dominant hand, pick up the catheter. Hold the distal end of the catheter loosely coiled in the palm of the dominant hand. • Insert the indwelling or straight catheter. • Ask the patient to bear down gently as if to void. Then slowly insert the catheter through the urethral meatus • Advance the catheter until urine flows through the tubing. • After visual confirmation of urine return into the tubing, continue to advance the catheter.
