chief complaint:" I haven't been feeling good, I am so tired, and i have gained so much weight lately" Patient is a 45 y/o female, who came to discuss test results, she indicates started with complaints of persistent fatigue, unexplained weight gain, and a noticeable decrease in energy levels. She also mentions feeling unusually cold, experiencing dry skin, and having some hair thinning. Her menstrual cycles have become irregular over the past few months, blood test results showed elevated cholesterol levels,- TSH: Elevated and- Free T4: Low, we started on levothyroxine and we will evaluate response in 6 weeks ICD 10  E03.9
